REQUEST FOR PROPOSALS
MARSHALL MUNICIPAL UTILITIES
MARSHALL, MINNESOTA
TO WHOM IT MAY CONCERN:
Marshall Municipal Utilities of the City of Marshall, Minnesota will
receive sealed proposals until 3:00 p.m. on Friday, June 24, 2016 for the following:
Group Life Insurance
Proposals shall be made on the forms provided by Ochs, Inc. and be submitted to Marshall Municipal Utilities and Ochs, Inc. via US mail in a securely sealed wrapper and clearly marked upon the outside
indicating “SEALED LIFE PROPOSAL -Marshall Municipal Utilities”. One copy of the quotations shall be directed to each of the following:

Proposals shall be in accordance with the specification information now on file at the office of Ochs, Inc. Request for proposal forms and
specification information can be obtained by calling Emily Watters of Ochs, Inc. at (651) 665-3789. Emily can also be reached via e-mail at ewatters@ochsinc.com.
Quotations will be received until 3:00 p.m. on Friday, June 24, 2016 at the offices of Marshall Municipal Utilities and Ochs, Inc. Proposals should be based on an effective date of January 1, 2017. No formal opening of proposals will occur.
Marshall Municipal Utilities reserves the right to reject any or all
proposals, and to make an award which is deemed most favorable and advantageous to the employees and MMU, regardless of whether or not such proposal is lowest.
